APPOINTMENTS CONFIDENTIALITY When Disclosure is Required by Law: Some of the circumstances where disclosure is required by the law are where there is a reasonable suspicion of child, dependent, or elder, abuse or neglect and/or when a client presents a danger to self, to others, to property, or is gravely disabled. When Disclosure May be Required: Disclosure may be required pursuant to legal proceedings. If you place your mental status at issue in litigation initiated by you, the defendant may have the right to obtain the psychotherapy record and/or therapeutic testimony. In couple and family therapy, or when different family members are seen individually, confidentiality and privilege may not apply between the couple or among family members. Clinical judgment will be used when revealing such information. Records will not be released to any outside party unless authorization to do so is obtained by all adult family members who were part of treatment. TELEPHONE CONSULTATION & EVALUATION PROCESS The first two to three sessions are used to evaluate, assess, and develop collaborative treatment goals for your work in therapy. Payment is due prior to the start of each session. Insurance is not accepted directly; however, an itemized invoice will be provided on a monthly basis that can be submitted for reimbursement. Because insurance companies set the rules for reimbursement, your individual insurance company will determine your eligibility for reimbursement. It is recommended that you apply for reimbursement through your personal insurance.
Appointments need to be rescheduled and cancelled at times. Please call as soon as possible. There is a 48-hour cancellation policy which means if an appointment is not rescheduled or cancelled 48 hours before the appointment, you will be responsible for payment of the full fee and the appointment will be charged to your account.
